Job Description Seasons Hospice & Palliative Care Inc. is a community-based organization with an ongoing mission to find creative solutions that add quality to end-of-life-care. With locations throughout the greater Chicagoland area, we recognize that individuals and families are the experts in their own care and we continuously strive for excellence beyond accepted standards. We are seeking an experienced Assistant Director of Clinical Services dedicated to the patient/family focus of hospice and palliative healthcare delivery.
As the Assistant Clinical Director you will assist the Director of Clinical Services in planning, developing, implementing and evaluating all patient/family clinical services for the hospice. You will also:

• Direct the day-to-day operations and management of all clinical services for our Willowbrook, Ill location.
• Maintain ongoing quality assurance activities related to patient and family services, including QI studies and chart audits etc.
• Participate in state(s) and national hospice organization activities.
• Assure state and federal regulatory compliance applicable to hospice and palliative care services.
• Act as a liaison between staff, patients, physicians, and other health care providers which promote and support a culture of service excellence.
• Continually monitors clinical episode management and provide direction/redirection and staff education as necessary.
• Ensure delivery of quality care to patients consistent with the mission of Seasons Hospice.
• May perform patient assessment, planning, and implementation of the plan of care.
• Must be able to work independently in directing patient needs by virtue of his/her experience and the competencies required for this position.
Qualifications Qualification/Requirements:
Graduate of an Accredited School of Nursing; BSN preferred
Current Registered Nurse licensure in the State of Illinois
Current CPR certification.
A minimum of three to five years of management experience, preferably in hospice or home health setting.
Previous experience in hospice, home health, oncology and geriatrics or general medical- surgical nursing required.
CHPN certified preferred.
Strong focus on customer service required.
Strong computer skills preferred
