| Job Description |
Seasons Hospice is currently looking for caring and compasionate LPN's to work in our Continuous Care program. Under the direct supervision of a registered nurse, the LPN will provide continuous care services, in-home respite care services, and personal care for the patient and assists with activites of daily living.
We currently looking to fill a full time and a resource position |

| Qualifications |
. Licensed by the state of Illinois as a Licensed Practical Nurse.
2. Current CPR certification
3. Previous experience in hospice, home health, geriatrics, or general medical/surgical nursing.
4. Demonstrates a commitment to the hospice philosophy of care.
5. Must have reliable automobile, valid driver’s license, and the minimum state required liability auto insurance.
