Job Description The Admissions Nurse-RN works 4-10 hour shifts. Days and hours may vary.

The Admissions Nurse provides nursing assessment, initial plan of care and intervention(s) that optimize patient and family directed comfort and quality of life.

Duties include but are not limited to:

1. Receives assignments from the Team Director or Referral Department for evaluation of appropriateness, sign up and or assessments for care.

2. The Admissions Nurse will formulate the Interdisciplinary plan of care and communicate to the Team Director.

3. Obtains data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.

4. Instructs the patient/family on procedures for contacting hospice.

5. Makes initial contact with attending physicians, other hospice team members and other agencies as needed to commence care and use of resources for the patient/family.

6. Maintains daily communication with the Team Directors or Customer Care Department.

7. Turns in all paperwork pertaining to sign ups and assessments within 24 hours to Clinical Services.

8. Maintains confidentiality of sensitive information.

9. Works in conjunction with the Director of Business Development and attends Hospice Care Consultant meetings, working to develop and strengthen provider relationships as part of that team.

10. Flexible scheduling of 40-hour salaried week.

11. Participates in hospice orientation and ongoing education programs.

12. Demonstrates familiarity with policies of the agency and state(s) and federal regulations pertaining to nursing services.

13. Participates on committees and special projects and other related duties as assigned.
Qualifications 1. Registered Nurse currently licensed in the state(s).

2. Previous experience in hospice, home health, geriatrics or general medical surgical nursing.

3. Demonstrated commitment to Seasons Hospice’s philosophy of care, values, mission statement and organizational culture.

4. Willingness to work a flex schedule of 40 hours a week and serve patients/families in the geographic service area.

5. Detail oriented with good time management skills.

6. Above average documentation skills.

7. Management/leadership skills.

8. CPR Certified

9. Must have reliable automobile, valid driver’s license, and the minimum state required liability auto insurance.
