| Job Description |
As a reflection of Seasons Hospice Admission policies, the Admissions Nurse performs pre-admission evaluations for appropriateness for hospice services. Initiates the admission process by educating and informing patient/family of hospice and its services. Also obtains informed consents. Provides nursing assessment, initial plan of care and intervention(s) that optimize patient and family directed comfort and quality of life. Must have hospice experience.

| Qualifications |
Registered Nurse curently licensed in the state.
Previous experience in hospice,home health,geriatrics or general medical surgical nursing.
Demonstrated commitment to Seasons Hospice's philosphy of care, values, mission statement and organizational culture.
Willingness to work a flex schedule and serve patients/families in the geographic service area.
Detail oriented with good time management skills.
Above average documentaion skills.
Management/leadership skills.
12hr shift Sat and Sun
rotating 8hr shift during week |
