RN Case Manager
Job Description:
Seasons Hospice has an opportunity for compassionate RN's who want to make a difference in the lives of patients and families coping with end of life illnesses.
Looking for RN's to care for patients in Oakland, Wayne and Washtenaw counties. You will provide, develop and implement individualized plans of care to maximize comfort and assist the family in the care. Additionally, you will be performing nursing assessments on new and ongoing patient/families, as well as assisting in coordinating care with the interdisciplinary team.
This position is full-time, Monday through Friday. Sharing in an on-call rotation for weekends is expected.
Documentation is electronic. Some computer experience is helpful, but not required. Will train.

| Qualifications |
Registered nurse currently licensed in the state of Michigan.
Current CPR certification preferred
Previous experience in hospice, home health, geriatrics or general medical surgical nursing
Must have reliable automobile, Driver’s License and the minimum state required liability auto insurance
